Online doctor consultations in South Africa: what to expect and when they help

Online care can be a useful first step for many non-emergency concerns, but it is not a replacement for physical examination or emergency services when those are needed. Doorstep starts with the patient’s need and routes to virtual, home or in-person care according to safety and suitability.

What an online consultation can do

A virtual consultation can support history-taking, symptom review, medication questions, follow-up and clinical decision-making when a remote assessment is appropriate. It may also lead to a prescription, certificate, test request or referral when an authorised clinician decides that is clinically appropriate.

Doorstep does not require patients to decide whether they need a nurse or doctor before assessment. The platform can begin with a care request and route the next step according to the information provided, urgency and location.

  • Medication and chronic-care follow-up
  • Results discussion and next-step planning
  • Escalation to home or in-person care when examination, observations or procedures are needed

When virtual care is not enough

Some concerns require physical examination, observations, diagnostic testing or urgent treatment. Severe chest pain, major breathing difficulty, stroke symptoms, loss of consciousness, major bleeding or other life-threatening symptoms should not wait for a routine online consultation.

In South Africa, emergency medical assistance can be reached on 112 from a mobile phone or 10177 for ambulance response.

Professional and privacy expectations

South African health professions are regulated. Doorstep’s operating model separates directory presence from professional and activity-specific verification before deployment. Telehealth must also preserve confidentiality, appropriate records and professional accountability.

Patients using Doorstep keep their care journey in one account so that bookings, messages, results and follow-up can remain connected rather than being scattered across unrelated services.

Questions people ask

Can an online doctor prescribe medicine in South Africa?

An authorised clinician may prescribe when it is legally and clinically appropriate after assessment. Doorstep does not treat prescription medicine as a direct-to-consumer retail product.

Can I get a sick note from an online consultation?

A clinician may issue an appropriate medical certificate when the consultation and professional rules support it. A certificate is not automatic simply because a consultation took place.

Can Doorstep arrange a home visit after an online assessment?

Where service availability and clinical suitability allow, Doorstep can route a patient from remote assessment into home nursing, diagnostics, a Health Desk or another in-person pathway.
